Stallone is in great shape for his age. I wouldn't rule out a little extra "help" though. But that's me being cynical.
I've got used to "Tango & Cash" image of Stallone. That's the bad part of being an actor. It's easy to notice you're getting old. Still Stallone is in great shape, my grandfather died at 66 and he was looking way older...
guys, the first one was okay. It was a very nostalgic-back-to-the-early-90-s-action film. You shouldn't expect something profound.
Neither should you this time. Its gonna be loads of guns, muscles and explosions. Thats it 
And I cant fucking wait!
Only thing that could have made this movie more badass is if Michael Bay directed it and Hans Zimmer scored it.
