Do you have a 360 controller plugged in? Check that or other input devices; unplugging my controller fixed it.

It was introduced in a semi-recent SDK update... no idea what happened, or if they've even been able to fix it since then. (I'll re-bug my man on the inside

my shortcuts became reliably unresponsive when i moved from vista 32 bit to win 7 64, and i don't have any controllers plugged in.
my solution is to just press every button 8 times in succession.
