You can enable blood in the gameplay options. For a reason I cannot fathom it's disabled by default.

It's quite a realistic implementation of blood, though. You get spurts if you land a headshot and there'll be a nice dark pool of blood beneath bodies, but don't expect crazy over-the-top gore.
