Hammer HDR Lighting Problem (black spots)

Annoying problem where black spots are appearing around the lit areas in my tunnels. I'm not seeing any leaks and moving around the light entities doesn't help. The "block light" brush I have above the model also seems to no longer have any effect. This is only happening when I compile with HDR. Light entities used are "light", "light_spot", and "env_lightglow".

Seems like there are too many different lightmaps calculated for each face. Are any of the light entities switchable, or have you given them a name? It's a classic cause of this issue.

You cannot have triggerable / named lights when compiling HDR in CS:GO, it's buggy and creates a checkerboard pattern, which seems to be the case here.
